Optimal Balance MD is a consultative practice, specializing in Functional Medicine for Women. Such services are provided separately from Dr. Harris’ obstetrics & gynecology practice, the Women’s Wellness Center.
Optimal Balance MD does not participate with any insurance carrier. This is because the consultative services provided by Optimal Balance MD, including Functional Medicine services and preventative wellness plans, are intended to supplement and enhance your healthcare experience. They are not services which are covered by Medicare or commercial insurance. Therefore, Optimal Balance MD does not bill insurance companies on your behalf, and will not provide diagnostic or procedural codes for such services.
Laboratory testing, particularly those tests which may be performed through private laboratory companies such as Quest or Labcorp, deemed necessary during the course of your evaluation and treatment may be covered by your plan. Billing for these services is handled directly by the laboratory. Diagnostic codes are provided to the laboratory by Optimal Balance MD. However, we cannot guarantee coverage for laboratory services. Optimal Balance MD has no control over, and therefore will not be held responsible for, any amounts billed to you by the laboratory. These charges may represent your deductible or coinsurance.
Specialized laboratory tests, such as adrenal function testing, are often not covered. This depends greatly on your insurance plan – for example, Blue Cross and United Healthcare plans require us to send to Quest or Labcorp, which do not offer many of these tests.
Optimal Balance MD will make every effort to inform you whether or not a test should be covered. If they are not expected to be covered, you will be charged directly by Optimal Balance MD. In these cases, you should not receive a bill from the laboratory for these tests. Optimal Balance MD will make every effort to inform you, to the best of our knowledge, which tests fall into this category, and which do not.
If, as a result of your consultation with Optimal Balance MD, you require traditional obstetric or gynecologic testing or treatment (i.e., an ultrasound or biopsy), such services should be obtained through your primary ob/gyn. If you wish to have these services performed by Dr. Harris through the Women’s Wellness Center, which offers general obstetric & gynecologic care, these appointments are scheduled separately from Optimal Balance MD. Most services provided by the Women’s Wellness Center are “in network” for most major insurance plans, including Medicare, as determined by your individual health insurance policy. Optimal Balance MD does not “double-book” patients for appointments. In other words, the time you schedule is yours alone. When you call to schedule the appointment, the amount of time necessary will be determined – this will range anywhere from 30 to 90 minutes.
If you are unable to be at your scheduled appointment, we request that you provide 24 hours of notice to cancel or reschedule. This allows us to free up the time for one of our other valued patients.
We will allow one (1) cancellation within the 24 hour time period free of charge. To discourage repeated late cancellations, the following fees will apply:
- Cancellation or rescheduling less than 24 hours before your appointment will result in a $50 charge applied to the credit card on file.
- After your appointment time has passed (or if you fail to arrive for your appointment), a $100 charge will be applied to the credit card on file.
Late arrivals will be offered the opportunity to use any remaining time within their scheduled appointment slot. This is your time with the doctor, and we want you to get the most out of your consultation!
